- Duly completed application Form (8) attested to by a Commissioner for Oaths
- Duly filled in Questionnaire
- 2 Passport size photographs
- Copy of marriage certificate
- Original certificate of good conduct
- Passport copy of applicant
- Copy of Kenyan Spouse`s Passport /ID
- A joint Sworn in Affidavit
- Copy of dependent’s pass/permit
